How do I schedule a visit?

What should I bring to my first visit?
Your photo ID, your insurance card if you have one, a list of your medicines and any recent dental X-rays. Health history forms are completed in our private portal or on paper at the office, not on this website.
What if I have a toothache or a broken tooth?
Call us right away and we'll fit you in the same day when we can. If you have swelling that spreads to your face, eye or neck, or trouble breathing or swallowing, call 911 or go to an emergency room.
Is parking available?
Yes. There's a lot behind our building, and street parking along McHenry Avenue.
