What this page covers

This page explains what Dentin Dental asks for through this website, what we do with it and what we keep private.

What the contact form collects

Your name, email, phone, how you'd like us to reply, a good time to reach you and a short message. We use it to answer you and to set up a visit.

What it leaves out

A contact form is no place for your health history or symptoms, so please keep them out of your message. If something is urgent, call us. Health history and consent forms are completed in our private patient portal or on paper at the office.

Your patient records

We keep your records private and share them only as the law allows. The notice of privacy practices you receive at your first visit explains your rights and how to use them.

Who sees your information

Only the staff who need it to answer you or care for you. We don't sell your information. We share it with a lab, a referring or receiving dentist, or your insurance plan only when it's needed for your care or your claim, or when the law requires it.

Your choices

You can ask to see, correct or delete a message you sent us, and you can ask us to stop contacting you by email or text. Contact us and we'll tell you what we can do.
